We have informed the employee (or his principal/deputy etc) to visit you at the Minnan Hospital providing the relevant medical diagnosis documents（□for outpatient：disease certificate and outpatient medical record；□for inpatient: hospitalization certificate or discharge summary）to (① obtain a medical review from your doctor; ② obtain approval to transfer to other hospital).
