论 著 文章编号 1006－8147（2012）04-0474-04 咀嚼肌间隙病变的 CT 及 MRI 诊断 张 鹏，张 敬，张云亭 （天津医科大学总医院放射科，天津 300052） 摘要 目的：探讨发生在咀嚼肌间隙（MS）病变的影像特征及其诊断与鉴别诊断。方法：对 31例 MS病变的影像学表现进行回 顾性分析，29例经手术和病理证实，2例经临床诊断。结果：19例原发病变中，下颌骨鳞癌 3例，血管瘤 4例，感染 7例，软骨肉 瘤、骨软骨瘤、含牙囊肿、表皮样囊肿和成釉细胞瘤各 1例。12例继发病变中，上颌骨鳞癌 1例，颈动脉鞘间隙血管瘤 2例，颈动 脉鞘间隙神经鞘瘤 1例，颌下腺区多形性腺瘤 1例，腮腺多形性腺癌 1例，颈动脉鞘间隙脂肪瘤 1例，鼻咽纤维血管瘤 1例，右 眼眶及右鼻窦非角化性癌 1例，左中前颅底脑膜瘤 1例，腮腺肌上皮癌 1例，鼻咽癌 1例。结论：CT和 MRI能清晰显示 MS病 变的形态、边界、范围、相邻结构的异常改变,对鉴别咀嚼肌间隙炎症与肿瘤、继发恶性肿瘤的临床分期有重要价值。 关键词 咀嚼肌间隙；计算机体层成像；磁共振成像 中图分类号 R445 文献标志码 A CT and MRI diagnosis of diseases involving the masticator space ZHANG Peng, ZHANG Jing, ZHANG Yun-ting （Department of Radiology, General Hospital, Tianjin Medical University, Tianjin 300052, China） Abstract Objective: To study the imaging features of primary and secondary diseases involving the masticator space in order to differ－ entiate them. Methods: CT and MRI findings of 31 cases with diseases arising from masticator space confirmed pathologically and clini－ cally were retrospectively studied. Results: In the 31 cases, 19 were primary diseases including 3 squamous cell carcinomas of mandible, 4 hemangiomas, 7 inflammations, and 1case in each of chondrosarcoma, osteochondroma, dentigerous cyst epidermoid cyst, and adaman－ toblastoma. 12 were secdonary diseases including 2 hemangiomas from carotid space,1 in each of squamous cell carcinoma of maxillae, pleomorphic adenoma from submandibular space, polymorphous adenocarcinoma from parotid, lipoma from carotid space, fibroangioma from rhinopharynx, non-keratinizing carcinoma from orbit and sinus maxillaris, meningioma, myoepithelial carcinoma. Conclusion: CT and MRI can clearly manifest the lesion and its extension in masticator space, differentiate inflammation from tumors,and provide informa－ tion for clinical management.
